Qualifications

• Two years of licensed clinical experience in rehab or related field.
• Rehabilitation, LTACH, acute care, or healthcare marketing/business development preferred.
• Strong communication, relationship-building, organization, and presentation skills.
• Ability to work collaboratively with clinical and business teams.
• Results-oriented mindset focused on helping patients and families navigate care.

Responsibilities

• Build and maintain strong referral relationships within an assigned territory.
• Serve as a clinical resource and representative of PAM Health to patients, families, physicians, case managers, discharge planners, nurses, and other partners.
• Complete clinical assessments of prospective patients to determine appropriateness for admission.
• Cultivate new referral opportunities and maintain existing referral relationships.
• Analyze referral and admission trends to identify opportunities for growth.
• Collaborate with the Admissions Manager and admissions staff to facilitate referral conversion and admissions.
• Partner with nursing, therapy, physicians, and other internal teams to support seamless transition of care.
• Educate patients, families, and referral sources about PAM Health's programs and capabilities.
• Develop and maintain relationships with healthcare stakeholders including physicians, ICU and floor nurses, case managers, discharge planners, payer representatives, patients and families.
• Provide feedback to support program development, quality improvement, and customer service.
• Monitor patient and referral source satisfaction and identify opportunities to improve the experience.
• Represent PAM Health professionally throughout the community and assigned territory.
