Job summary

Mount Sinai Morningside is seeking a Patient Services Coordinator to support delivery of healthcare services in our practice. The role acts as a liaison for patient care activities, educates patients about their care plans, and coordinates services with Primary Care Providers and outpatient testing facilities.

You will work with the care team to ensure timely appointments, track follow-ups and reminders, and help resolve issues affecting patient progression.

Qualifications

• Some college; Bachelor’s degree preferred.
• 3 years healthcare/customer service or relevant experience.

Responsibilities

• Facilitate patient care plans by coordinating appointments and information during daily huddles and case discussions.
• Collaborate with clinical staff to resolve issues impacting patient care progression.
• Monitor dashboards and organize daily tasks, including visit reminders, follow-ups, and authorization tracking.
• Contact insurance carriers for pre-certifications and discuss patient needs with the Practice Manager.
• Provide front-end coverage and route telephone encounters to the appropriate Providers.
• Act as liaison among Physicians, patients, and staff to connect patients with internal/external resources.
• Suggest process improvements to enhance patient satisfaction.
• Perform other duties as assigned.
