Job summary

Fortis Care is seeking a Care Coordinator in Suwanee, GA for an on-site, hourly role with a Monday–Friday schedule, 8:00 a.m.–5:00 p.m. You will partner with providers and care teams to coordinate patient-centered care, manage transitions, and prevent hospitalizations.

The role emphasizes education, follow-up, and a concierge, patient-first approach in a high-quality rehabilitation setting. LPN/MA experience and EHR familiarity are preferred.

Qualifications

• LPN or MA required.
• 1–2 years experience in care coordination or related settings.
• Associate’s degree or higher in clinical/health field preferred.
• Experience with care coordination documentation and EHR systems preferred.

Responsibilities

• Performs outbound calls to understand clinical needs and connect resources.
• Serves as a warm, responsive point of contact for patients and families.
• Follows up with patients to ensure needs are met and schedules future check-ins.
• Monitors hospitalizations and follows up with care team to gather information.
• Coordinates care transitions and post-hospital follow-up; educates on discharge instructions.
