Fortis Care, in partnership with Sadie G. Mays Health & Rehabilitation Center, seeks a mission-driven Care Coordinator to join the on-site clinical team in Atlanta. Qualifications

• Licensed Practical Nurse (LPN) or Medical Assistant (MA) required.
• 1–2 years of clinical experience in primary care, skilled nursing, long-term care, ambulatory care, chronic care management, or hospital setting.
• Familiarity with chronic conditions, preventive care, and patient education.
• Experience with an EHR or care management platform preferred.
• Associate’s degree or higher in a clinical/health-related field preferred.

• Serve as a warm, responsive point of contact for patients and families, coordinating needs and following through to resolution.
• Proactively connect with patients and families to understand clinical needs, provide updates, and schedule follow-up.
• Coordinate with physicians, specialists, pharmacies, hospitals, and other healthcare partners to schedule appointments and obtain records.
• Identify and prioritize patients needing attention each day, build provider schedule, and prepare charts for efficient care.

Mays has served the community with compassion, dignity, and excellence. We are looking for a mission-driven care coordinator to join our team.

The Care Coordinator is a central member of the on-site clinical team, helping ensure that patients with complex medical needs receive coordinated, responsive, and thoughtful care.

Working closely with our Care Coordination Manager, Advanced Practice Provider, physicians, and facility team, you’ll help organize each patient’s care, anticipate needs, and make sure nothing falls through the cracks. You’ll build relationships with patients and families, coordinate appointments and transitions, prepare the clinical team for efficient visits, and follow through on the details that can make the difference between a patient remaining stable in the facility and ending up back in the hospital.

This is an on-site, hourly role with a predictable Monday–Friday, 8:00 a.m.–4:00 p.m with significant flexibility to work remotely in the afternoons, with no evening or weekend requirements. It’s an ideal opportunity for an LPN or experienced Medical Assistant who enjoys patient relationships, thrives on organization and follow-through, and wants to play a meaningful role on a collaborative clinical team.

Key Responsibilities

• Serve as a warm, responsive point of contact for patients and families, answering questions, coordinating needs, and ensuring every request is followed through to resolution.
• Proactively connect with patients and families to understand clinical needs, provide updates, and schedule ongoing follow-up.
• Coordinate with physicians, specialists, pharmacies, hospitals, and other healthcare partners to schedule appointments, obtain records, and keep care moving forward.
• Help identify and prioritize patients who need medical attention each day, build the provider schedule, and prepare charts so clinicians can deliver efficient, high-quality care.
• Partner closely with the on-site Advanced Practice Provider, facility nursing, therapy, social work, and other team members to keep care and communication seamless.
• Monitor hospitalizations and emergency department visits and ensure the Fortis Care team has the information needed for timely follow-up.
• Support transitions of care after hospitalization, including arranging follow-up, confirming medication reconciliation, and reinforcing discharge instructions.
• Provide patients and families with appropriate educational materials and assist with communication related to specialty referrals and appointments.
• Track open items and future follow-ups to ensure patient needs are addressed promptly and consistently.
• Document all care coordination and care management activities accurately and in a timely manner.

Qualifications

• At least 1–2 years of experience in a clinical setting such as primary care, skilled nursing, long-term care, ambulatory care, chronic care management, or a hospital environment.
• Familiarity with chronic medical conditions, preventive care, and patient education.
• Experience working in an EHR or care management platform preferred.
• Associate’s degree or higher in a clinical or health-related field preferred.
• Prior care coordination, quality improvement, or population health experience is a plus.
• Experience in a concierge, hospitality-oriented, or other high-service environment is also a plus.
