Job summary

Fortis Care is seeking a Care Coordinator to support patients with complex medical needs. This on-site, hourly role offers a Mon-Fri 8:00 a.m.–5:00 p.m. schedule and works closely with CCM management, providers, and care teams to prevent hospitalizations and improve outcomes.

The ideal candidate is an LPN or MA with 1–2 years in care coordination, strong communication, and solid EHR experience. Expect collaborative teamwork in a premier Northeast Atlanta facility.

Qualifications

• Licensed Practical Nurse (LPN) or Medical Assistant (MA) required.
• 1–2 years in care coordination or related clinical settings.
• Familiarity with care management and EHR systems preferred.

Responsibilities

• Coordinate care for patients with complex needs under supervision.
• Make outbound and inbound calls to patients and providers.
• Engage patients and families with education and follow-up.
• Document activities accurately in the EHR.
• Collaborate with on-site care teams to ensure seamless transitions.
