Job summary

Fortis Care is seeking a Care Coordinator to support patients with complex medical needs in a premier Northeast Atlanta facility. This on-site, hourly role runs Monday–Friday from 8:00 a.m.

to 5:00 p.m., with no weekends or evenings, and focuses on patient engagement, care planning, and proactive follow-up to prevent hospitalizations. You will work with patients, families, and care teams, handling calls, coordinating appointments, documenting activities in the EHR, and ensuring smooth

Qualifications

• LPN or MA certification required.
• 1–2 years in care coordination or related fields.
• Associate degree or higher preferred.
• Experience with EHR systems preferred.

Responsibilities

• Performs outbound calls to understand patient needs and connect to resources.
• Serves as a warm, responsive point of contact for patients and families.
• Follows up with patients to ensure needs are met and schedules future check-ins.
• Monitors hospitalizations and coordinates with care team and resources.
• Provides education materials and coordinates enrollments/appointments.
• Identify and prioritize patients for medical services daily.
• Coordinate post-hospital follow-up, medication reconciliation, and discharge education.
• Collaborate with Advanced Practice Provider, nursing, therapy, and social work teams.
• Document all care management activities accurately in the system.
