Mobile Medical is seeking a Patient Care Navigator in Jacksonville, FL to coordinate patient care after visits and ensure timely follow-up, medications, referrals, and services are obtained.

Qualifications

• Minimum one year of medical office or healthcare administrative experience.
• Experience with patient scheduling and front office operations.
• Strong customer service and communication skills.
• Excellent organizational and multitasking abilities.
• Comfortable working in a fast-paced, high-volume environment.
• Proficiency with Microsoft Office.

Responsibilities

• Coordinate patient care after provider visits and ensure medications, referrals, and services are arranged.
• Process provider orders and follow-up tasks.
• Coordinate Home Health, lab, radiology, and DME services.
• Complete medication and service prior authorizations.
• Process prescription refill requests.
• Communicate with patients, providers, pharmacies, and specialists.
• Schedule and coordinate follow-up care.
• Maintain accurate documentation within the EMR.
