Job summary

COLORADO HEALTH NETWORK INC is seeking a Clinical Patient Navigator to support PrEP and HIV/HCV programs at the Denver clinic. The role involves eligibility assessments, insurance assistance, health education, and coordinating referrals to medical and social services.

Under the Clinical Services Officer, you will manage an active caseload, provide outreach and monitoring, and maintain accurate EHR documentation. Bilingual preferred; evenings/weekends as needed.

Qualifications

Responsibilities

  • Coordinate and maintain an active caseload of PrEP and HIV/HCV patients.
  • Assist with access to medications and adherence support.
  • Enter case notes and maintain timely data entry in the EHR.
  • Serve as liaison between patients and care teams.

Patient Navigation services will support CHN to better serve those disproportionately at risk for or living with HIV/HCV by improving their access to medical care. Bilingual preferred.Essential Job Functions:Provide screenings, outreach and initiatives to promote PrEP, HIV/HCV programCoordinate and maintain an active caseload of PrEP, HIV/HCV patientsProvide support and guidance to individuals seeking PEP by supporting their transition into PrEPPerform assessments, plan creation, and assist patients with access to medication and adherence to treatment as well as risk reduction plansResponsible for tracking, monitoring and occasional reporting of patient activities, including access adherenceComplete necessary paperwork and data entry in a timely fashion including entering case notesProvide appropriate patient referrals to both internal and external agenciesCollect necessary documentation required for program complianceAssist patients as they move through the service delivery systemAssist with the elimination of barriers including, but not limited to: insurance, financial housing and transportationProvide emotional support for patients and their families, as neededAct as a liaison between patient and all relevant professionals, programs, and resourcesEducate patients and others about HIV disease process and preventionAssist with, and coordinate, community education and outreach initiatives as requestedMaintain and upkeep of electronic health records (EHR)Participate and attend staff, department, team and community meetings upon requestPerform effectively within the organization and interpersonally with co-workersDevelop positive working relationships with other social service, medical and behavioral health providersParticipate in fund-raising and advocacy eventsMust be available to work evenings and weekendsMust be able to work in a diverse setting with diverse populations, including persons living with HIV, the LGBTQ+ community, persons of various ethnic backgrounds, disenfranchised communities.Participation in training related to HIV, substance use, harm reduction, trauma informed care and related issues.
