Job summary

Eskenazi Health seeks a PrEP Care Coordinator to educate patients on PrEP/PEP and HIV risk reduction, navigate insurance, and connect with PrEP prescribers across the Marion County network.

The role requires collaboration with internal teams and external partners, maintaining records and coordinating same-day initiation and follow-up within 90 days. Bilingual preferred with Indiana Navigator licensure possible.

Qualifications

• Bachelor’s degree preferred in a relevant field or a high school diploma with related coursework in human behavior/social policy/counseling.
• Spanish and English proficiency preferred.
• Licensure as Indiana Navigator or eligible.
• Positive demeanor with excellent communication skills.

Responsibilities

• Facilitate education on PrEP/PEP, HIV risk reduction, and insurance navigation.
• Link patients to PrEP prescribers and community partners.
• Assess insurance status and assist with coverage and financial navigation.
• Coordinate same-day PrEP initiation and follow-up appointments within 90 days.
• Maintain thorough records of referrals and communications.
• Engage in outreach events and participate in program meetings.
