Patient Care Navigator (Fox Chase Cancer Center)

The Patient Care Navigator provides access to quality care for patients diagnosed with cancer. In collaboration with registered nurses, the Navigator builds trusting relationships with patients through guidance, engagement, coordination of care, resource allocation, and addressing questions for the healthcare team. The role serves as a point of contact to support patients throughout their multidisciplinary cancer treatment, assisting with illness perception, care coordination, and appointment scheduling. The Navigator works in alignment with the nurse navigation department, demonstrating a service-oriented approach, team collaboration, and interaction with co-workers and caregivers. Staying current with organizational and community resources relevant to the patient population is essential.

Minimum Requirements

• High School Diploma or Equivalent (Required)
• Bachelor's Degree (Preferred)
• Completion of a Patient Care Navigator education program (preferably from George Washington University or American Cancer Society) (Preferred)

Experience

• 2 years experience in a healthcare setting or equivalent, preferably in oncology (Required)
• Knowledge of basic clinical concepts (Required)
• Experience with EPIC systems (Preferred)
• Understanding of complex patient issues and social determinants of health (Preferred)
• Experience with patient outreach methods and strategies (Preferred)
• Medical terminology knowledge (Preferred)
• Experience working as a patient navigator or community health worker (Preferred)
