Sr Talent Acquisition Partner @ Temple Health

The Patient Care Navigator functions to provide access to quality care for patients diagnosed with cancer. In conjunction with the registered nurse, the Patient Care Navigator builds trusting relationships with patients through guidance, engagement, coordination of care, resource allocation, and assists with questions for the healthcare team. The Patient Care Navigator serves as a point of contact to patients, assisting with support throughout the multidisciplinary nature of cancer treatment. The navigator works in alignment with the nurse navigation department, assisting the team in areas such as illness perception, care coordination, and appointment assistance. The Patient Care Navigator must demonstrate a model of service, team collaboration, and be willing to interact with other co-workers and patient's caregivers. This role includes staying current with organizational and community resources for the patient population served.

Education

• High School Diploma or Equivalent (Required)
• Completion of Patient Care Navigator education program, preferably from George Washington University or American Cancer Society (Preferred)

Experience

• 2 Years experience in a healthcare setting or equivalent, preferably in oncology (Required)
• Knowledge of basic clinical concepts (Required)
• Knowledge of EPIC systems (Preferred)
• Understanding of complex patient issues and social determinants of health (Preferred)
• Experience with patient outreach methods and strategies (Preferred)
• Knowledge of medical terminology (Preferred)
• Experience working as a patient navigator or community health worker (Preferred)
