Position Overview

The Patient Care Navigator provides access to quality care for patients diagnosed with cancer. Working alongside registered nurses, the Navigator builds trusting relationships through guidance, engagement, care coordination, resource allocation, and addressing questions from patients and their families. The role involves supporting patients throughout their multidisciplinary cancer treatment, serving as a point of contact, and staying current with organizational and community resources.

Responsibilities

1. Assist patients with understanding their diagnosis and treatment options.
2. Coordinate care and appointments in collaboration with healthcare teams.
3. Support patients in navigating healthcare and community resources.
4. Build trusting relationships with patients and caregivers.
5. Stay informed about organizational resources and community services.

Qualifications

• High School Diploma or Equivalent (Required)
• Bachelor's Degree (Preferred)
• Completion of a Patient Care Navigator education program (Preferred)
• Minimum 2 years experience in healthcare, preferably in oncology (Required)
• Knowledge of clinical concepts, EPIC systems, and social determinants of health (Preferred)
• Experience as a patient navigator or community health worker (Preferred)
• Knowledge of medical terminology and patient outreach strategies (Preferred)
