Job summary

The Population Health Nurse Navigator at The Ohio State University serves as a care coordination expert within a multidisciplinary team. You will advocate for patients, coordinate navigation across value-based care programs, and connect patients and families with necessary services to improve outcomes and access.

You will manage high-risk populations, support follow-up and primary care connections, and help divert avoidable hospital utilization through community resources and home-based care

Qualifications

• Bachelor’s Level Degree in Nursing from a CCNE or ACEN accredited program required.
• Current Ohio RN licensure.
• Minimum of three years of recent clinical experience preferred; five years in primary care, case management, or post-acute settings highly desirable.

Responsibilities

• Coordinate patient-centered care and education across the healthcare continuum.
• Advocate for patients and navigate value-based care, CICIP-funded programs, and home-based/post-acute models.
• Facilitate timely follow-up and primary care connections; support chronic and behavioral health needs.
• Divert avoidable inpatient/ED utilization by linking to outpatient, community, and home-based services.
