Job summary

El Centro Family Health in Las Vegas, NM seeks a Care Coordinator to help patients navigate services, set goals, and adopt healthy behaviors while coordinating with Medical, Behavioral, and Dental teams for integrated care.

You will handle appointment scheduling, patient intake, referrals to community resources, and outreach, maintaining HIPAA compliance and delivering compassionate support to diverse patients and families.

Qualifications

• High school diploma or GED required.
• Minimum of two years' experience, with at least one year in a related field.
• Proficient with Microsoft Office and EMR systems.
• Excellent telephone etiquette and customer service.

Responsibilities

• Answer phones, schedule, reschedule, and cancel appointments.
• Assist with Medicaid/insurance applications and enrollment processes.
• Coordinate with medical providers for coordinated care.
• Provide referrals to community services and advocate for patients.
• Maintain confidentiality and HIPAA compliance.
• Assist with patient intake interviews and outreach.
