We seek a Patient Navigator to connect patients with care teams, provide information and support, and help overcome barriers to receiving timely care.

Competenze

• High School diploma or equivalent with 2 years related experience.
• Completion of healthcare-related vocational program with 1 year related experience.
• Higher education may substitute for experience.
• Comparable combination of education and experience may substitute for the above requirements.

Mansioni

• Provide individualized, patient-centered care and support.
• Link patients with care providers to facilitate services and empower patient involvement.
• Help patients overcome barriers to disease management using available resources.
• Maintain navigation records and protect patient confidentiality and privacy.
• Organize patient schedules and coordinate appointments across the care team.
