Responsibilities

• Provide clinical oversight to a specific vulnerable patient population within a department.
• Serve as intermediary between service lines to ensure the patient care plan remains intact and patient-centered.
• Assist the provider in medical management of outpatients in the clinic.
• Participate in direct patient care activities including assessments, vital signs, procedures, injections, wound care, and phlebotomy.
• Manage calls and messages from patients, families, providers and pharmacies to address needs promptly.
• Educate patients and caregivers about ongoing care plans, medications, and follow-up schedules.
• Coordinate care across disciplines and help overcome barriers to care including transportation and referrals.
• Monitor care plans and ensure smooth transitions in-care and post-discharge.
• Adhere to infection control and safety standards; attend required trainings.
