Job summary

CCHC is seeking a Care Coordinator to join the care management team. You will assist with outreach to patients, gather medical records, and support the care management model across the care team in accordance with CCHC policies.

You will monitor MHN portal, conduct initial screenings, educate patients, and coordinate with Medical Home to develop individualized care plans, while addressing barriers to adherence and ensuring timely appointments.

Qualifications

• High school diploma or equivalent required.
• Three years of experience in a medical office; ambulatory experience required.
• One to three years of experience with electronic medical records.
• Medical Assistant program completion preferred.
• Knowledge of the needs of highly complex patients.
• Strong verbal and written communication skills and interpersonal skills.
• Good organizational skills.
• Ability to multi-task and be self-motivated.
• Ability to work effectively independently and as part of a team.

Responsibilities

• Monitors MHN portal and coordinates care activities.
• Gathers and delivers required documents for the care management model.
• Assists with initial patient screening, surveys, and education under the care team guidance.
• Follows up with patients in hospital/clinic settings as needed.
• Audits patients’ records and collaborates with Medical Home to arrange care.
• Acts as a liaison to multiple departments and providers.
• Connects patients to community-based services and referrals within insurance plans.
• Schedules appointments and monitors adherence to appointments.
• Documents correspondence in MHN portal and EMR systems.
• Develops individualized care plans with the care management team.
• Identifies barriers to adherence and works with patients to alleviate them.
