Job summary

This entry-level role supports patients and families with educational information and coordinates with care providers, administration, and payers to sustain effective care plans. Responsibilities include coordinating with the healthcare team, educating patients about options and resources, and promoting self-advocacy.

Qualifications

• High School Diploma.
• 1 year minimum experience in care coordination / case management (preferred).
• Experience with Athena Health EHR (preferred).
• Community Health Worker (CHW) (preferred).
• Bilingual English/Spanish (preferred).
• Certified or Registered Medical Assistant (preferred).
• Certified Coder preferred.
• Current Florida Certified or Registered LPN, active and in good standing (preferred).
• Completion of accredited Certified or Registered Medical Assistant program (preferred).

Responsibilities

• Plan with the patient, family/caregiver, primary care provider, support staff, payers, and the community resources to maximize healthcare outcomes.
• Facilitate communication and coordination among health team members and involve the patient in decision-making.
• Educate the patient and family about treatment options, benefits, and care coordination needs.
• Empower the patient to problem-solve by exploring options and alternative plans.
• Encourage appropriate use of healthcare services and strive for cost-effective care.
• Assist in safe transitioning of care to the next appropriate level.
• Promote patient self-advocacy and self-determination.
• Advocate for patient to improve outcomes for patient, team, and payers.
• Attend monthly meetings with insurance payers and support quality improvement goals.
• Assist referral tracking and send reminders for pending referrals.
• Other duties as assigned.
