Job summary

Rendr in New York City is seeking a Patient Care Navigator to coordinate patient services and ensure high-quality, personalized care across our clinics. You will screen patients by phone, document encounters in the EMR, verify insurance, and help schedule providers while maintaining clear communication among patients, families, and clinicians.

Ideal candidates have a High School Diploma (Bachelor's preferred), bilingual English/Chinese, and experience with MDLand or EMR systems.

Qualifications

  • High School Diploma or equivalent, bachelor’s degree preferred.
  • 1 year of work experience in a medical practice, mental health experience is a plus.
  • Knowledge in medical terminology
  • Excellent verbal and interpersonal communication skills
  • Familiarity with MDLand and EMR data entry/retrieval
  • Able to work with pressure
  • Customer service orientation, compassionate personality
  • Bilingual in English and Chinese (Mandarin or Cantonese) is required

Responsibilities

  • Follow the practice protocol and coordinate patient services to ensure comprehensive, personalized care.
  • Conduct initial screening and document encounters in the EMR per physician guidelines.
  • Verify insurance eligibility and benefits and help schedule providers to ensure timely care.
  • Obtain medical records from internal and external sources and track medication history.
  • Maintain patient charts and ensure accuracy of information in the EMR.
  • Follow up with patients to monitor progress and adherence to treatment plans.
  • Facilitate communication between patients, families, and healthcare providers.
  • Assist with generating letters and forms for services and address inquiries per physician guidelines.

Position: Patient Care Navigator

Job Responsibility
  1. Follows the practice protocol, coordinates patient services for patient member as needed, ensure they receive comprehensive and personalized care.
  2. Conducts initial screening and brief assessment with patient on phone encounters under physician guidelines; maintains documentation of all patient encounters in the EMR system.
  3. Collaborates with medical receptionist to verify patient's insurance eligibility and benefits and organizes schedules for providers to meet patients. Ensure patients receive a caring service in a timely manner.
  4. Requests and obtains medical records of patient members from various internal and external parties, including but not limited to, primary care providers, specialists, hospitals and pharmacies. Track controlled substance medication history.
  5. Scans, uploads, and attach documents to patient charts correctly, assists to maintain the accuracy of patient information in the EMR system.
  6. Follows up with patient to monitor condition and adherence to treatment plans under physician guidelines. Reports to providers with patients’ progress.
  7. Facilitates smooth and effective daily communication between the patient, family members, and healthcare providers to ensure patient satisfaction and quality of care.
  8. Supports patient care by e.g. generating patient letters and referral letters, filling out forms for special services etc.
  9. May assist to address patient’s medical inquiries, medication concerns and complaints under physician guidelines.
Min Requirements
  1. High School Diploma or equivalent, bachelor’s degree preferred
  2. 1 year of work experience in a medical practice, mental health experience is a plus
  3. Knowledge in medical terminology
  4. Excellent verbal and interpersonal communication skills
  5. Demonstrated ability to multi-task and use multiple systems, including MDLand, familiarity with data entry and retrieval in Electronic Medical Records (EMR) systems
  6. Able to work with pressure
  7. Customer service orientation, always shows care, kind and passionate personality
  8. Bilingual in English and Chinese (Mandarin or Cantonese) is required
