Heart to Heart Hospice is seeking a Patient Care Manager - Registered Nurse in Terre Haute, IN. The role leads nursing staff, coordinates clinical departments, and ensures continuity, quality, and safety of services in compliance with participation standards.

The ideal candidate holds an active Indiana RN license, a valid driver's license, and at least two years of hospice RN experience. Strong organization, communication, and multidisciplinary leadership are essential.

Qualifications

• Active state RN license as required for practice.
• Valid driver's license, reliable transportation, and current auto liability insurance.
• Minimum two years as a Hospice RN in clinical care or home health.
• Motivated, self-directed, flexible, able to work in a multidisciplinary group.
• Strong organizational, people, and problem-solving skills with multi-tasking ability.
• Excellent critical thinking, organization, interpersonal and communication skills.

Responsibilities

• Direct and coordinate clinical departments and sites; ensure continuity, quality, and safety of services in compliance with regulations.
• Supervise nursing staff; provide direction to ensure Plan of Care adherence and continuity of services by qualified personnel.
• Support education to enhance knowledge and skills; train staff to maintain high patient care quality.
• Coach, develop, provide feedback, and conduct annual evaluations of team members.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state, and federal laws and Medicare regulations; follow personnel policies.

What You Must Have:

• Current State Registered Nurse License
• Current State Driver’s License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as a Hospice Registered Nurse in a clinical care setting, or home health
• Must be highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary group setting
• Possess good organization, people and problem solving skills and the ability to multi-task many projects and strategies simultaneously
• Strong critical thinking, organizational, interpersonal, and communication skills

What You Will Do

• Direct and coordinate clinical departments and sites; assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations (Conditions of Participation).
• Supervise and provide direction to nursing staff and team members, in an effort to ensure quality, compliance with Plan of Care, assessment and reassessment of patient's needs and continuity of services by appropriate health care personnel.
• Supports employee and patient education to enhance knowledge, skills and achieve quality experiences. Continuously trains nursing staff to retain employees with the highest quality of patient care.
• Provide coaching, development, feedback and annual evaluations to team members in a professional and timely manner.
