Job summary

RareMed Solutions is seeking a Care Liaison to serve as the primary contact for incoming calls and prescription referrals under a patient service program. You will assist with intake, data entry, referral processing, triage of inquiries, and follow-up with patients and caregivers.

You will work cross-functionally with Case Management and Pharmacy triage teams, ensuring SOPs and service levels are followed while maintaining a strong privacy mindset.

Qualifications

• Associates Degree or high school diploma with equivalent work experience.
• 2+ years’ experience in healthcare management setting.
• Empathy, drive and commitment to exceptional service.
• Strong analytical skills with attention to detail.
• Ability to work flex schedule per Program business need.

Responsibilities

• Execution of end-to-end processes to support data entry and referral triaging.
• Day to day phone support to route calls and resolve inquiries.
• Ensure referrals are processed and triaged per SOPs.
• Interface with patients and caregivers to coordinate prescription delivery.
• Provide feedback to operations to correct issues promptly.
• Maintain awareness of QA measures and program rules.
