Responsibilities

• Coordinate care with Primary Care Providers and outpatient services to support patient care plans.
• Educate patients on their care plans and coordinate services across departments.
• Facilitate appointments, track follow-ups, and assist with prior authorizations and billing issues.
• Act as a liaison among physicians, patients, and clinical staff to connect patients with internal/external resources.
• Identify opportunities to improve processes and patient satisfaction.
