Job summary

UBHCC Crisis Center in Poplar Bluff, MO is seeking a full-time Care Coordinator to support individuals and families affected by mental illness, substance use disorders, and related conditions. You will provide education, assist with service access, and help clients develop skills for recovery and independence.

The role requires a degree in a human services field and relevant experience; you will document care, coordinate transportation when needed, and connect clients with community resources

Qualifications

• Qualifying Experience must include serving individuals with mental illness, SUD or developmental disabilities.
• Experience includes one-on-one or group services with recovery/resiliency focus.
• Assist individuals to achieve goals on their treatment plan.

Responsibilities

• Provide education to individuals about mental illness and/or substance use disorders.
• Provide transportation for individuals when medically necessary.
• Link individuals with community resources and develop natural supports.
• Develop or improve interpersonal, social, family, community and independent living skills.
• Complete required documentation in a timely manner.
• Adhere to evidence-based practice models and program policies.

Education

Master's or Bachelor's degree in a human services field

Associate of Applied Science in Behavioral Health Support

Bachelor's degree unrelated to human services with two years qualifying experience

Four years of qualifying experience
