Job summary

Silverado Hospice in Orange County is seeking a Patient Care Coordinator to ensure seamless coordination of patient care and support clinical teams. You will be the key link between patients, families and hospice leadership, upholding quality care and compliant operations.

Responsibilities include managing clinician schedules, reviewing documentation for accuracy, and assisting in interdisciplinary group conferences while ensuring regulatory compliance at local, state and federal levels.

Qualifications

• LVN with valid state license.
• Minimum 1 year experience in a clinical healthcare setting.
• Experience in administrative healthcare settings required; hospice preferred.
• Background in home health, healthcare, or hospice is preferred.

• Manage clinical associates’ schedules, including holidays, vacations, weekends, and daily assignments.

• Ensure compliance with local, state, and federal regulations and hospice standards.
• Assist in planning and implementing interdisciplinary group (IDG) conferences.

What You’ll Be Doing

• Reliable transportation to meet attendance and punctuality standards (driver’s license not required unless specified)

Schedule: Monday - Friday, 8:00am - 5:00pm

Anticipated pay range $29 – $31 USD
