Job summary

Silverado Hospice San Diego is seeking a compassionate Clinical Liaison to guide patients and families from pre-admission through discharge, bridging clinical, sales, and marketing teams to optimize transitions into hospice care.

The role emphasizes empathy, relationship-building, and collaboration with healthcare providers to grow referrals while aligning with Silverado’s mission and values. A valid driving record is required and travel to service areas is expected.

Qualifications

  • Must have RN or LVN license (preferred, not required).
  • Experience with hospice patients and families is highly preferred or demonstrated by volunteering.
  • Demonstrated effective communication and interpersonal skills.
  • Must pass a criminal background check, physical exam, and drug screening.

Responsibilities

  • Serve as an advocate for patients from pre-admission through discharge from medical facilities.
  • Collaborate with sales, marketing, and clinical teams to source hospice referrals from healthcare providers.
  • Review Explanation of Benefits (EOBs) and consent forms with patients and families.
  • Perform comprehensive nursing assessments to evaluate hospice eligibility.
  • Support the Hospice service line in developing and executing sales and marketing strategies.
  • Promote Silverado’s mission and values through community engagement and public relations.

We’re hiring aClinical Liaison (Hospice) – a compassionate and collaborative professional who serves as an advocate for patients and families during the pre-admission process. This role bridges clinical, sales, and marketing teams to ensure a smooth transition into hospice care and supports the growth of our service line through strategic outreach.

What You’ll Be Doing

  • Valid driver’s license and clean driving record required
  • Must comply with all state and federal driving regulations
  • Reliable transportation required
