Lorian Health, a leader in home health care, is seeking a Patient Care Coordinator in Las Vegas. This position supports referral intake and manages clinical staff schedules while ensuring timely patient care.

Qualifications

• Working knowledge of medical terminology.
• Strong knowledge of Microsoft Office systems (i.e., Word, Excel) and EMR systems, HCHB is preferred.
• Must have professional and customer-service-driven phone and communication skills.
• Ability to organize and prioritize workload based on incoming referrals.
• Confidentiality of patient information in compliance with HIPAA standards.

Responsibilities

• Support reception and intake of new referrals.
• Maintain clinical field staff schedules for admissions and discharges.
• Act as a liaison between Lorian and referral sources.
• Enter patient information into HomeCare HomeBase system to meet MEDICARE requirements.
