Madison Medical seeks a Patient Care Coordinator to lead the patient journey from first contact through ongoing care. This person serves as the central point of communication between patients, providers, and internal teams, ensuring a smooth, professional, and well-coordinated experience at every touchpoint.

The Patient Care Coordinator serves as the primary point of contact for new and returning patients and coordinates scheduling across providers with clinical awareness. This role manages check‑in, check‑out, and intake workflows, guides patients through onboarding and care plans, and helps patients understand insurance benefits and out‑of‑network coverage.
This person also coordinates referrals, maintains accurate documentation, supports compliant record‑keeping in CureMD, and works across Outlook, Microsoft Teams, and RingCentral. The role includes tracking patient progress through internal systems and reporting tools, partnering with clinical and billing teams to improve patient flow, protecting patient confidentiality, and staying calm and solution‑focused in high‑volume settings.
Candidates should have at least five years of experience in patient‑facing roles within hospitals or private practice settings. Strong knowledge of insurance and out‑of‑network care models is required, along with experience coordinating multidisciplinary care. Proficiency with EMR systems and Microsoft platforms is essential, as are strong organizational and communication skills.
