Job summary

Humana's CenterWell Home Health seeks a Patient Care Coordinator to coordinate care with hospitals, nursing facilities, and discharge planners to determine patient eligibility and ensure smooth transitions.

You will conduct facility visits, support the clinical team in eligibility determinations, and help process physician orders while reporting to a Market Executive of Sales. Travel within the territory is required and you will be patient-facing.

Qualifications

• Must have RN/LPN/LVN license.
• Bachelor's of Science in Nursing preferred.
• 3 years clinical experience preferred.
• Knowledge of governmental regulations, Medicare eligibility requirements, and patient care planning.
• Excellent customer service, account development, organization, time management, problem-solving, communication and selling skills.
• Travel within an assigned territory and to sales meetings as required.
• Role is patient facing and part of TB screening program.

Responsibilities

• Develop and maintain contact with hospital, skilled nursing, and discharge planning services to provide updates on Humana's services.
• Primarily conduct facility visits to assist program clinical team in determining patient eligibility.
• Communicate and collaborate with program staff and field sales resources via weekly meetings; participate in business development meetings.
• Assist program in processing physician orders as directed.
