Job summary

Humana's CenterWell Home Health is seeking a Patient Care Coordinator to manage discharge planning coordination with hospitals and facilities, and to support the clinical team in determining patient eligibility.

You will travel within your territory, build relationships, and ensure timely communication with program staff. This patient-facing role requires an RN/LPN/LVN license and a nursing degree preferred, with 3 years clinical experience.

Qualifications

• RN or LPN or LVN license required.
• Bachelor's of Science in Nursing preferred.
• 3 years clinical experience preferred.
• Knowledge of governmental regulations and Medicare eligibility requirements.
• Strong customer service, organization and communication skills.

Responsibilities

• Develop/maintain contact with discharge planning services at hospitals and facilities.
• Conduct facility visits to assist program clinical team in determining eligibility.
• Communicate with program staff and field resources via weekly meetings.
• Assist program in processing physician orders as directed.
• Report to a Market Executive of Sales.

Skills
