Job summary

CenterWell Home Health, part of Humana, is seeking a Patient Care Coordinator to coordinate care across hospitals and SNFs, meet patients at the bedside, and support operations and sales efforts. The role requires nursing licensure and travel within the territory.

Ideal candidates have clinical experience, knowledge of regulations and Medicare requirements, and experience with EMR systems Watershed or Olio. 40 hours weekly with full-time benefits.

Qualifications

• LPN, LVN or RN license required.
• Knowledge of governmental regulations and Medicare eligibility requirements.
• Three years clinical experience preferred.
• Experience with EMR systems Watershed and Olio highly preferred.
• Travel within assigned territory required.

Responsibilities

• Work with local sales team and report to Market Executive of Sales.
• Coordinate care from Hospitals and SNFs in the territory.
• Meet patients at bedside to discuss home health services and expectations at home.
• Support operations by ensuring resources, information, and coordination for start of care.
• Assist with sales calls to support Account Executive and address client needs.
