Job summary

CenterWell Home Health seeks a Patient Care Coordinator to build and maintain relationships with hospitals, SNFs and discharge planners, supporting eligibility determinations and care coordination for patients. The role involves facility visits at physician request and close collaboration with the program clinical team.

Applicants should have an active RN/LPN/LVN license, a BSN preferred, and about 3 years of clinical experience with strong knowledge of Medicare requirements and care planning

Qualifications

  • RN/LPN/LVN license required.
  • Bachelor's of Science in Nursing preferred.
  • 3 years clinical experience preferred.
  • Must have strong knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
  • Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills.

Responsibilities

  • This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $59,300 - $80,900 per year
