Job summary

Software Placement Group is seeking a Patient Care Coordinator in a fast-paced, on-site role in New York. You will manage a caseload, coordinate caregivers, and ensure timely authorizations and care plans while maintaining open communication with patients, families, hospitals, and MLTCs.

You will monitor utilization, resolve scheduling issues, and work closely with internal teams to keep services uninterrupted and compliant with plans of care. Benefits are included with a full-time schedule.

Responsibilities

• Manage an assigned caseload of patients and oversee day-to-day coordination of services.
• Ensure patients are staffed with reliable caregivers who meet schedule, needs, and plan of care.
• Proactively address staffing issues to prevent gaps in care.
• Monitor utilization percentages and ensure authorized hours are staffed.
• Maintain regular communication with patients and representatives regarding status and satisfaction.
• Communicate with MLTCs and payors regarding services and authorizations.
• Escalate urgent issues to management when necessary.
• Maintain accurate patient schedules and records in agency systems.
• Build strong relationships with patients and caregivers, maintaining professionalism.
