A home health care provider is seeking a Patient Care Coordinator in Philadelphia County, PA. This entry-level position involves onboarding patients and caregivers, ensuring compliance with policies, and fostering communication with families for quality service. Ideal candidates are bilingual, have a strong dedication to helping others, and possess strong communication skills. No prior home-care experience is required as comprehensive training will be provided, ensuring a supportive and fulfilling role in care delivery.

• Ensure compliance with law and company policy.
• Communicate with patients, caregivers, and families for quality service.
• Assist with on-site visits, as needed.

Responsibilities

• Screen new patients and caregivers
• On‑board new patients, including providing assistance with any documentation/clearance requirements
• On‑board new caregivers, including orientation/training
• Staff patient cases with caregivers and ensure patient cases are started promptly
• Ensure compliance with the law and company policy, including caregiver clock‑in and clock‑out requirements
• Communicate with patients, caregivers, and patient families to ensure satisfaction and quality service delivery
• Assist with on‑site visits, as needed
