White Glove Community Care is seeking a Patient Coordinator to join our home care team. You will coordinate home care cases, manage caregiver scheduling, and serve as the main contact for patients and families.

Qualifications

• 2+ years experience in home care, healthcare coordination, patient services, or medical office setting preferred.
• Strong knowledge of home care operations, scheduling, or case coordination.
• Experience with HHA Exchange, EVV systems, or healthcare EMR platforms is a plus.
• Excellent communication, multitasking, and problem-solving skills.
• Ability to manage high-volume workload and prioritize effectively.
• Bilingual (Spanish/English) preferred.

Responsibilities

• Coordinate and manage home care cases and caregiver scheduling.
• Serve as the primary point of contact for patients, families, and caregivers.
• Handle high-volume calls, follow-ups, and patient inquiries.
• Ensure accurate documentation and compliance in HHA Exchange.
• Collaborate with nurses, case managers, and clinical teams to support patients' needs.
• Monitor attendance, coverage, and missed visits.
• Maintain daily, weekly, and real-time updates on case status.
• Provide excellent customer service in a fast-paced healthcare environment.
