Some of the most important people in healthcare aren't the ones providing hands-on patient care. They're the ones making sure nothing falls through the cracks!
The missing physician signature. The hospital discharge that needs coordination. The patient waiting for an update. The medical record that needs to be tracked down.
At Summit Home Care and Home Health Care, Inc., we're looking for a detail-oriented, resourceful Healthcare Operations Coordinator who enjoys solving problems, staying organized, and keeping things moving.
For over 30 years, our organizations have helped patients receive quality healthcare in the comfort of their homes. Now we're looking for someone who can help our clinical teams deliver that care efficiently!
If you're currently working in a medical office, handling patient calls, coordinating referrals, or managing healthcare documentation, you already know how important the behind-the-scenes work is.
This position gives you the opportunity to take those skills beyond a traditional front-desk role and become an essential part of a larger healthcare operation.
You'll work directly with clinical teams, help coordinate patient transitions, and solve everyday challenges that can delay care.
Keep Patient Care Moving: Collect, organize, and track medical records, physician orders, and essential patient documentation.
Connect With Patients: Complete pre-admission, five-day follow-up, and discharge calls to ensure patients feel supported.
Solve Documentation Challenges: Track down missing signatures, face-to-face documentation, and physician orders.
Coordinate Hospital Transitions: Monitor hospitalized patients, obtain H&P and Resumption of Care documentation, and communicate discharge updates.
Support Your Team: Collaborate with Community Liaisons, nurses, and clinical staff to ensure timely coordination of care.
High school diploma or equivalent required; college education preferred.
At least one year of experience in a healthcare office, medical administrative role, or fast-paced professional environment.
Strong organizational skills and the ability to manage multiple priorities.
Excellent phone communication and customer service skills.
Typing proficiency of 50+ words per minute.
Familiarity with medical terminology preferred.
Experience with patient coordination, medical records, referrals, or healthcare customer service is a plus!
Predictable Schedule: Monday through Friday, 8:00 AM–5:00 PM, with no regularly scheduled weekends.
Meaningful Work: Your organization and attention to detail directly support patients receiving care at home.
Team Environment: Work alongside experienced healthcare professionals who value communication and collaboration.
Career Development: Build valuable experience in healthcare operations, patient coordination, and clinical administration.
Established Organization: Join a team with over 30 years of experience serving Minnesota patients and families.
Maybe you're a Patient Care Coordinator, Medical Receptionist, Referral Coordinator, Patient Access Representative, Medical Records Specialist, Healthcare Administrative Assistant, or Medical Office Coordinator looking for something more.
If you enjoy staying busy, solving problems, helping people, and being the person who keeps everything organized, we'd love to meet you!
