Caremed Inc. is seeking an experienced Patient Care Coordinator (Home Care Scheduler) to join our home health operations team. This critical role is responsible for end-to-end patient care coordination, caregiver scheduling, Electronic Visit Verification (EVV) compliance, and daily client service delivery.

Caregiver & Patient Scheduling: Oversee daily roster management, matching home health aides and caregivers with client care plans across Central Virginia.
Shift & Coverage Management: Manage open shifts, call-outs, schedule changes, urgent cancellations, and immediate reassignments to ensure uninterrupted patient care.
Electronic Visit Verification (EVV): Perform real-time EVV tracking, visit verification, attendance reconciliation, and resolve EVV exceptions for billing compliance.
On-Call Support: Participate in a rotating on-call schedule for weekend and after-hours emergency dispatching and care coordination.
Interdisciplinary Communication: Serve as the primary liaison between patients, family members, field caregivers, RN Supervisors, and administrative management.
Documentation & Compliance: Maintain precise, HIPAA-compliant patient files, authorization records, and visit logs within care management software platforms.
Education: High School Diploma or GED required; Associate’s or Bachelor’s degree in Healthcare Administration, Business, or related field is a plus.
Experience: 2+ years of experience in healthcare scheduling, medical office administration, home health coordination, or patient services.
Technical Skills:
Proficiency in Electronic Visit Verification (EVV) software and Electronic Health Records (EHR).
Strong working knowledge of Microsoft Office Suite (Word, Excel, Outlook, Teams).
Experience with Medicaid authorizations, Plans of Care (POC), and home care compliance standards.
Soft Skills: Outstanding phone etiquette, conflict resolution, crisis management, multitasking, and organizational skills in a fast-paced environment.
