Platinum Home Health Services in Cleveland is seeking a Patient Care Coordinator to serve as the central connection among referral sources, patients, families, physicians, and clinical teams. You will help coordinate the patient journey from referral through admission with timely communication, accurate documentation, and insurance verification.

This on-site role requires Sunday availability with a weekday of your choice; join a team focused on exceptional home health care and a positive patient

Qualifications

• High school diploma or equivalent is preferred.
• Experience in healthcare intake, scheduling, or care coordination is a plus.
• Strong organizational and communication skills required.

Responsabilités

• Receive referrals from hospitals, physicians, and facilities.
• Verify patient demographics, insurance, and documentation.
• Coordinate Start of Care visits within time frames.
• Communicate with patients and families about services and scheduling.
• Collaborate with nurses, therapists, billing and referral sources.
• Maintain EMR documentation and HIPAA compliance.
• Monitor referrals and resolve barriers to timely admission.
• Assist with scheduling adjustments across clinical teams.

Position Summary

As a Care Coordinator, you'll serve as the central connection between referral sources, patients, families, physicians, and our clinical team.

You'll help coordinate the patient journey from referral through admission while ensuring timely communication, accurate documentation, insurance verification, and seamless scheduling.

Your work helps ensure patients receive the care they need when they need it, making you an essential part of our mission to deliver exceptional home health services.

This position requires Sunday availability. The Care Coordinator will select a weekday of their choice.

Responsibilities

• Receive and process referrals from hospitals, physicians, facilities, and community partners.
• Verify patient demographics, insurance coverage, authorizations, physician orders, and required documentation.
• Coordinate Start of Care visits within required time frames.
• Communicate with patients and families regarding services, scheduling, and expectations.
• Collaborate with nurses, therapists, scheduling, billing, physicians, and referral sources to ensure seamless care coordination.
• Maintain accurate documentation within the electronic medical record (EMR).
• Monitor referral progress and proactively resolve barriers to timely admission.
• Assist with scheduling adjustments and coordination across clinical disciplines.
• Maintain compliance with HIPAA, Medicare, Medicaid, and agency policies.
• Provide exceptional customer service while creating a welcoming experience for every patient and referral source.

Qualifications

• High school diploma or equivalent preferred.
• One to two years of healthcare intake, scheduling, care coordination, or administrative experience preferred.
• Strong organizational and multitasking skills.
• Excellent communication and customer service abilities.
• Proficiency with Microsoft Office (Outlook, Teams, Word, Excel, and PowerPoint).
• Comfortable learning electronic medical record (EMR) systems.
• Ability to prioritize competing responsibilities while maintaining attention to detail.
• Passion for helping others and supporting exceptional patient care.
