As a Care Coordinator at Ohioans Home Healthcare, your highly organized nature and detail-oriented approach are key assets in coordinating daily care for patients. In this administrative position at our Perrysburg offices, you’ll schedule and manage the daily workload of caregiver staff as you coordinate care for patients.
You will apply your strong assessment skills to evaluate patients referred to the agency, coordinate care between all disciplines and ensure the integrity of patient care and the quality experience we’re known for. We’ll rely on you to assign cases to RN case managers, manage staff productivity and play a key role as an informational resource for nurses in the field.

We will monitor medication compliance, set-up, IV therapy, injections, and provide the education necessary for administering injections.
Our team ensures compliance with special diets, weight monitoring, and the utilization of intravenous nutrition such as TPN and tube feeding.
Includes instruction on the use of inhalers, aerosol machines, oxygen, and the proper care of all equipment.
Our team provides post-operative care, offering continual monitoring of chest pain, blood pressure, edema, and education on signs and symptoms of further cardiac problems.
We conduct dressing changes, wound vac therapy, assess wound healing and identify signs of infection. We also provide dressing supplies and order wound vacs as necessary.
The team will inform patients about the disease process of chronic obstructive pulmonary disease, congestive heart failure, diabetes, renal failure, multiple sclerosis, osteoporosis, and more.
Our experienced team of pediatric clinicians have the training to care for infants and children with a variety of diagnoses and special needs. Our team performs many pediatric skills including, PICC care, tracheostomy care, chemotherapy, parenteral nutrition, nasogastric or enterostomy feedings, medication administration, providing education to parents/caregivers, and many more.
