Job summary

Patriot Home Care, located in Beaver Falls, PA, is seeking talented and passionate individuals to join their team as Patient Care Coordinators. The role requires you to screen and onboard new patients and caregivers, ensuring satisfaction and compliance with company policies.

Applicants should have a strong work ethic, effective communication skills, and a passion for helping those in need. Training will be provided, and both associate’s and bachelor’s degrees are preferred.

Qualifications

• No homecare experience necessary – we will provide you with all the training you need!

Responsibilities

• Screen new patients and caregivers.
• On-board new patients, including providing assistance with any documentation/clearance requirements.
• On-board new caregivers, including orientation/training.
• Staff patient cases with caregivers and ensure patient cases are started promptly.
• Ensure compliance with the law and Company policy, including caregiver clock-in and clock-out requirements.
• Communicate with patients, caregivers, and patient families to ensure satisfaction and quality service delivery.
• Assist with on-site visits, as needed.
