Job summary

Talentify is seeking dedicated Patient Care Coordinators to join our homecare team serving patients across Pennsylvania and Delaware. You will screen new patients and caregivers, onboard patients, and coordinate caregiver scheduling to ensure timely care and satisfaction.

No homecare experience is required; we provide comprehensive training, a collaborative work environment, competitive pay, and full benefits.

Qualifications

• High school diploma or GED required; associate's or bachelor's preferred.
• No homecare experience necessary — we provide training.
• Bilingual preferred; strong communication skills required.

Responsibilities

• Screen new patients and caregivers.
• On-board new patients, including documentation/clearance requirements.
• On-board new caregivers, including orientation/training.
• Staff patient cases with caregivers and ensure timely start.
• Ensure compliance with law and Company policy, including clock-in/out.
• Communicate with patients, caregivers, and families to ensure satisfaction.
• Assist with on-site visits as needed.

Responsibilities

• Screen new patients and caregivers
• On-board new patients, including providing assistance with any documentation/clearance requirements
• On-board new caregivers, including orientation/training
• Staff patient cases with caregivers and ensure patient cases are started promptly
• Ensure compliance with the law and Company policy, including caregiver clock-in and clock-out requirements
• Communicate with patients, caregivers, and patient families to ensure satisfaction and quality service delivery
• Assist with on-site visits, as needed
