Housing Works, Inc. seeks a Patient Care Coordinator in Downtown Brooklyn to screen patients, verify insurance, and provide medical case management for Primary Care patients.

The role supports care coordination, PRAPARE assessments, referrals, and documentation in the EMR. Responsibilities include coordinating intake screenings, collaborating with care teams, and participating in quality improvement meetings.

Qualifications

• Bachelor’s Degree preferred.
• Strong understanding of HIV/AIDS, case management, and entitlements.
• Proficiency in Microsoft Office Suite.
• Solid knowledge of HIPAA regulations regarding medical document requests.
• Strong organizational and clinical skills are essential.
• Spanish proficiency and EMR experience preferred.

Responsabilités

• Coordinate intake screenings to determine patient eligibility for services.
• Work with Intake Specialist to ensure PRAPARE assessments are completed.
• Ensure new patient leads are generated in eICare.
• Administer PRAPARE assessments and provide referrals for Primary Care clients.
• Coordinate with care management for referrals to sub-specialty services.
• Participate in interdisciplinary meetings and transcribe minutes.
• Document hospitalizations and schedule discharge appointments.
• Collaborate with Program Partners and clinical staff on referrals.
• Organize on-site meetings with case managers to review client status.
• Maintain documentation in EMR and support clinical staff.
