Housing Works, Inc. is seeking a Care Coordination Specialist (CCS) in New York to deliver comprehensive case management for Primary Care patients.

You will coordinate enrollments, streamline referrals, document key patient interactions, and manage EMR workflows to ensure high-quality care for patients affected by homelessness, substance use, and HIV/AIDS. Qualifications

• Associate’s Degree required; Bachelor’s Degree preferred.
• 1–2 years in a medical setting or related field preferred.

Responsibilities

• Coordinate enrollment of uninsured patients with insurance programs (including ADAP).
• Assist patients with insurance authorizations and paperwork.
• Document hospitalizations and schedule discharge appointments.
