Job summary

CenterWell Home Health is seeking a Patient Services Coordinator to schedule visits and coordinate care between field staff, patients and physicians to ensure continuity of care. The role acts as a vital link between clinical staff and patients and their families, managing all aspects of a patient's healthcare maintenance and treatment.

The position requires at least 1 year of home health experience, strong organizational skills, and the ability to handle multiple schedules.

Qualifications

• 1 year of Home Health experience required.

Responsibilities

• Manage schedules for all patients including supervisory visits.
• Schedule discharge visits, OASIS collection, recert visits, and transfers.
• Reschedule declined or missed visits and reassignments.
• Prepare and edit weekly agent schedules and review productivity/geographic issues.
• Assist with weekly case conferences.
• Receive calls from field staff and refer clinical questions to Branch Director.
• Maintain the client hospitalization log and enter coordination notes.
• Send electronic logs to office, field and sales staff; review lab reports as needed.
• Verify visit notes in scheduling system and run scheduling reports.
• Assist with internal transfers between branches.
• Additional duties as needed by leadership.
