Job summary

CenterWell Home Health seeks a Patient Services Coordinator-LPN to schedule visits and maintain care coordination. You will communicate with field staff, patients, and physicians to ensure timely visits and continuity of care.

The role manages schedules, assists with office operations, and may involve on-call clinical tasks. Requires an active nursing license (LPN/LVN) and at least 1 year in home health, with strong organizational and communication skills.

Qualifications

• Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state.
• Minimum 1 year of home health experience.
• Packet review / QI experience preferred.
• Coding certification preferred.
• Valid state driver's license and automobile liability insurance.
• Current license in the state of employment if applicable.
• Excellent communication, organization, and multitasking abilities.

Responsibilities

• Schedule visits for patients and coordinate with field staff and physicians.
• Edit schedules for sick staff and reassign patients promptly.
• Maintain routine logs and assist with weekly case conferences.
• Process lab reports, route to medical records, and initiate infection reports.
• Support office and staff management and participate in on-call rotation if clinical.
• Run scheduling reports and prepare weekly schedules for approval.
