Job summary

CenterWell Home Health in St. Louis is seeking a Patient Services Coordinator 3 who serves as the vital link between clinical staff, patients and families.

You will schedule, organize and manage all aspects of a patient's healthcare maintenance and treatment, performing advanced administrative and operational support with independent judgment. You will coordinate scheduling for visits, transfers and field communications, maintain logs and generate scheduling reports while ensuring accurate

Qualifications

• 1 year of Home Health experience is required.
• Preferred packet review/QI experience.
• Coding certification is a plus.

Responsibilities

• Manage schedules for all patients, including supervisory visits.
• Schedule discharge visits, OASIS collection, recert visits, and TIF OASIS collections.
• Reschedule declined or missed visits and reassignments.
• Prepare and edit weekly schedules; review productivity and geography issues.
• Assist with weekly case conferences.
• Answer calls from field staff and route clinical questions to Branch Director.
• Maintain the hospitalization log and add coordination notes.
• Send electronic logs to office, field and sales staff.
• Verify visit notes in scheduling console as needed.
• Run scheduling reports (Agent Summary, Missed Visits).
• Assist with internal transfer of patients between branches.
• Additional duties vary by branch census and leadership discretion.
