Job summary

CenterWell Home Health is seeking a Patient Care Coordinator to join the local sales team and coordinate patient care across hospitals and SNFs within the territory. You will meet patients at the bedside to discuss home health services and what to expect at home, while supporting operations to ensure a successful start of care.

The role requires a licensed LPN/LVN/RN, knowledge of Medicare rules, regulatory understanding, and travel.

Qualifications

• LPN/LVN/RN license is required.
• Knowledge of governmental regulations and Medicare eligibility.
• Three years clinical experience preferred.
• Travel within assigned territory required.
• TB screening program participation may be required.

Responsibilities

• Coordinate care from Hospitals and SNFs in the territory.
• Meet with patients at the bedside to discuss home health services and expectations.
• Support operations by ensuring resources and information for start of care.
• Assist with sales calls to support Account Executive in client needs.
