Job Overview
The Patient Care Connect Coordinator manages patient registration, appointment scheduling, conducts eligibility functions, supports authorization retrieval, and handles inquiries to ensure smooth coordination of care across multi-specialties. Acts as a liaison to facilitate effective communication between patients, medical staff, and healthcare teams, ensuring a positive patient experience. Also, provides clerical support and other duties as assigned.
Essential Duties
- Clerical:
- Gather patient demographics, insurance info, and referral documentation for registration.
- Serve as a primary contact for patients and caregivers, ensuring a positive experience.
- Accurately enter data into computer systems using Care Connect functions.
- Verify insurance eligibility and authorizations, and explain coverage details.
- Identify financial resources and facilitate application processes.
- Respond to patient account inquiries.
- Maintain accurate records within the EMR system.
- Schedule appointments including labs, diagnostics, and consultations, accommodating patient needs.
Patient Coordination & Customer Service
- Guide patients through the Keck process, explaining procedures and providing directions.
- Coordinate communication and care between patients and healthcare providers.
- Address concerns and ensure effective information flow within the healthcare system.
- Interact courteously and professionally with patients and staff.
- Assist patients with online portal access and communication.
- Provide information on procedures, medications, and post-care instructions.
- Escort patients and provide directions within USC facilities.
- Help overcome barriers to care, such as translation and financial assistance.
- Act as a communication hub among patients, providers, and departments.
- Collaborate with departments for inter-clinical visits.
Teamwork & Collaboration
- Flexibly cover staff absences and work independently or in teams.
- Maintain open communication with management and colleagues.
- Perform additional duties as assigned.
Qualifications
- High School diploma or equivalent.
- At least 3 years of related experience in acute or ambulatory care settings.
- Experience navigating insurance benefits, verifying coverage, and explaining costs.
- Proficiency in Word, Outlook, and EMR scheduling.
- Excellent communication skills in English.
- Deep knowledge of medical insurance types and medical terminology.
- Strong analytical and problem-solving skills.
Preferred Qualifications
- Associate’s or Bachelor’s degree in a related field.
Licenses/Certifications
- Fire Life Safety Training (LA City): must be obtained within 30 days if not already held.
The hourly rate ranges from $22.00 to $34.18, considering experience, education, and other factors.
