Job Overview

The Patient Care Connect Coordinator manages patient registration, appointment scheduling, conducts eligibility functions, supports authorization retrieval, and handles inquiries to ensure smooth coordination of care across multi-specialties. Acts as a liaison to facilitate effective communication between patients, medical staff, and healthcare teams, ensuring a positive patient experience. Also, provides clerical support and other duties as assigned.

Essential Duties
  1. Clerical:
  2. Gather patient demographics, insurance info, and referral documentation for registration.
  3. Serve as a primary contact for patients and caregivers, ensuring a positive experience.
  4. Accurately enter data into computer systems using Care Connect functions.
  5. Verify insurance eligibility and authorizations, and explain coverage details.
  6. Identify financial resources and facilitate application processes.
  7. Respond to patient account inquiries.
  8. Maintain accurate records within the EMR system.
  9. Schedule appointments including labs, diagnostics, and consultations, accommodating patient needs.
Patient Coordination & Customer Service
  1. Guide patients through the Keck process, explaining procedures and providing directions.
  2. Coordinate communication and care between patients and healthcare providers.
  3. Address concerns and ensure effective information flow within the healthcare system.
  4. Interact courteously and professionally with patients and staff.
  5. Assist patients with online portal access and communication.
  6. Provide information on procedures, medications, and post-care instructions.
  7. Escort patients and provide directions within USC facilities.
  8. Help overcome barriers to care, such as translation and financial assistance.
  9. Act as a communication hub among patients, providers, and departments.
  10. Collaborate with departments for inter-clinical visits.
Teamwork & Collaboration
  1. Flexibly cover staff absences and work independently or in teams.
  2. Maintain open communication with management and colleagues.
  3. Perform additional duties as assigned.
Qualifications
  • High School diploma or equivalent.
  • At least 3 years of related experience in acute or ambulatory care settings.
  • Experience navigating insurance benefits, verifying coverage, and explaining costs.
  • Proficiency in Word, Outlook, and EMR scheduling.
  • Excellent communication skills in English.
  • Deep knowledge of medical insurance types and medical terminology.
  • Strong analytical and problem-solving skills.
Preferred Qualifications
  • Associate’s or Bachelor’s degree in a related field.
Licenses/Certifications
  • Fire Life Safety Training (LA City): must be obtained within 30 days if not already held.

The hourly rate ranges from $22.00 to $34.18, considering experience, education, and other factors.
