• Conducts telephonic or electronic follow-up to ensure continuity of care.
• Engages patients using motivational interviewing techniques.
• Grooms provider schedules to ensure appropriate visit interventions.
• Coordinates and attends patient case conferences with providers and follows up on actions.
• Updates provider goals and care plans for visibility across the care team.
• Completes care coordination orders and maintains a daily tracking system.
• Schedules provider appointments including primary care and specialists.
• Ensures discharge orders align with the care plan.
