Job summary

Qualifications

• RN license and acute nursing experience required or preferred.
• BLS certification required on hire.
• Certified Case Manager certification preferred (2 years).

Responsibilities

• Coordinate care and services from admission through discharge.
• Partner with interdisciplinary teams to develop and implement a comprehensive care plan.
• Monitor and evaluate options to meet healthcare needs and ensure safe transitions of care.
