• CPhT credential preferred.

Responsibilities

• Respond to member inquiries with timely, compassionate Care Guide support.
• Conduct proactive member outreach and education.
• Meet productivity, quality, and schedule adherence targets.
• Document outreach attempts and outcomes in approved systems.
• Escalate complex cases to Team Lead when needed.
• Maintain HIPAA compliance and data quality.
